Kitchen lighting dilemma for a vaulted ceiling

My kitchen is 13' x 20'. Dining space in one end. The ceiling is 8' high on the south outside wall, and is 18' on the north end (inside wall). House was built in the late 70's and the lighting sucks. All I have at this time is two sets of 2 can lights. We have to bring in a 14' step ladder to change the bulbs and my husband and I are in our 70's and can't carry the ladder let alone get on it anymore. I had a skylight cut in over the cooktop island a few years ago and that helps in the daytime, but at night the shadows drive us crazy. I also have an 8' breakfast bar which is dark. The ceiling over that is about `14' Pendant lights might work but I am just not sure about the esthetics of such a long chain or rod. Suggestions?
