Soil analysis help

I got my soil results back a little late and am already done leveling my backyard for planting but can still probably till in lime into my front yard before planting. My soil results were a pH of 4.3, high organic matter, nitrogen and phosphorus. Low in magnesium, Ca, K, Sulfur and NA [other micronutrients not tested]. I live around Seattle, WA, and will be planting tttf and yarrow around Sept. 7th. I don't need a perfect lawn but am worried about disease resistance. What would be the best plan? How much lime and how to apply in the front yard and how much for the backyard if I can't till it in? Also, should i wait until Nov. to fertilize? What would be a good choice? Thank you.
