Misunderstanding Medicare Part D "Annual Costs"

I ran the Medicare plan sorter for DH, entering his two daily meds -- one a Tier 3. The least expensive plan, rated by Annual Cost, doesn't seem to include the stated $400 deductible.

This plan's Premium is $22.50/mo ($270/annually). Deductible is $400. (They don't break out the cost of the meds, but meds are supposedly included in the calculations.) Annual Cost = $586. That's less than the premium + the deductible.

Also listed are two other plans, with higher premiums, but $0 deductibles. Their annual costs total $911 and $972.

Have I misunderstood how to figure a Deductible as Out Of Pocket expense. Is it ever not a part of Annual Cost?
