Help! Badly neglected apple tree with cankers/black rot.

This is a 30+ year old tree that's been neglected, but I'd like to do everything possible to save it going forward. All the symptoms point to black rot, but I'm open to other suggestions.

The tree wasn't been pruned properly as it grew, so it's more of a bush than an actual tree. Unfortunately, this makes removing the affected branches/trunks problematic.

Does anyone have any advice on how I should proceed? There's new and seemingly healthy growth coming from cankerous trunks, so should I try to shave away the infected bark and leave the trunk in place once the petals fall? Or just cut off all the infected sections? Also, are there any tips for cutting away the larger trunks/branches near the base? It's really difficult to identify a collar and a good place to cut, especially since it looks like the base itself is affected by cankers as well.

Thanks for any suggestions!
