You have a grafted Euphorbia. The top is a crested E. lactea variegata, not sure which Euphorbia the rootstock is. It is usually grown for the top, as the only purpose of the rootstock is to keep the top alive. But your rootstock has branched. These branches could be removed and rooted on their own. Succulent Euphorbia leaves for the most part are ephemeral, not lasting long on the plant for the most part. More information about what soil it is in, the amount of sunlight it receives and your watering practices would be beneficial in diagnosing the problem, if any.

Am I correct saying that the grafted top appears to be dead/dry?

Thank you, Christopher! The plant looked nearly the exact same for five years, not a single leaf fell off, and only a few grew. I am realizing now that the only change was that we had A/C installed recently and the vent comes down close to the plant. Perhaps it dried it out more quickly, or it did not like the cold air.

I water it once a week and it gets indirect sunlight. Unsure of the soil since it was a gift purchased from a specialty succulent store, but it has the white vermiculite looking rocks.

Sadly, the Euphorbia is almost certainly a goner. It started rotting in one small place inside and has now spread. Such a bummer.

p.s. rina_Ontario, CA: The grafted top is dead in one small spot on the top left, but otherwise has been thriving for the past five years looking exactly as it does in the photo.

I was asking since it looks so whitish to me. I really don't know them, had one once that didn't like me (R.I.P), but grafted top was more colorful. I did google some images, and majority has more color too.
I also thought that the rootstock is growing and maybe not supporting growth of the graft...Just wondering.

Rina, yes, I think you are right...The rootstock sending out branches is most likely a sign the top is not healthy.
