Need help reason cellulose blowing out of AC supply vents in house

Hi - I recently renovated a home, and the hvac contractor added an additional zone and duct work. Just turned it on for first time. When I turn on this zone, I get particles coming out of the supply ducts of the two closest rooms of the to the main house. I have periodic, whole pieces of cellulose insulation (which is what is used in attic) coming out onto one room. And in the other room, it is a constant stream of sand like particles raining down from the vent and over spreading the room on the desk. Needless to stay, I have turned off the HVAC until fixing the problem. Does anyone know what this might be? Note that I have 2 return vents. One 20 x 20 existing return in main house with a throwaway merv 12 filter. I have a another return vent in the addition that is 10 x 10 without any filter. And then a merv8 throaway filter in the actual air handler unit itself. I appreciate anyone's thoughts or insights. I needed some fresh air after that initial test, and hopefully can fix soon!
