Corn Plant/Cane Tree has seen better days

Hi all - Please help diagnose why my corn plant seems to be dying and what I can do to revive it. The pictures actually make it look better than it really does -- the shortest stalk is pretty much dead. I picked this up from a reputable nursery around March '14. It looked great at the time but is getting worse each day since around December. I'm in Chicago so it is currently pretty cold and we keep the house at 71 degrees. It's about 7 feet tall, 12 feet away from a east facing window and right against our west facing patio door. Although the door can be cold, there's not much of a draft and there is a heating vent right by it which runs virtually 24/7. I water at most once a week and 12-16 ounces each time. I often go 2 weeks in between waterings.

It came in some kind of coir, I added Black Gold soil right away when I repotted and recently added Fox Farms Ocean Forest. I've never put any kind of nutrient in it.

Since it's winter, it doesn't get much if any direct sun but these plants don't need much. I'd guess the pot it's in would hold around 6 gallons. Appreciate any expertise!
