Painting a Hoosier style cabinet

When I bought my Hoosier style cabinet, it was originally green and ivory. Then when we remodeled our kitchen three years ago, I painted the entire cabinet ivory. I'm still kicking myself for painting it. I wish I had left it with the original paint.

Now, some of the ivory paint has peeled off the bottom shelf and this is a perfect excuse to restore the cabinet to the original colors.

The doors were ivory and everything else was green (the base and top part). So, I can't take any piece of it that was green into a store for color matching. Does anyone have suggestions on the best way to match the green paint?

Also, what is the best way to paint it so the new paint doesn't peel off like this is doing?

Suggestions are very much appreciated! I want to take this back to how it looked, but I want to do it the right way so this doesn't happen again.
