Adoption Question

I was adopted in Tennessee in 1952. I located my birth parents in 1974 but we have never been very close. I just found out online that Tennessee law allows an adopted adult access to their adoption information and a copy of their original birth certificate. My birth certificate was issued after the adoption was final and lists my adoptive parents as the parents. I am trying to join the American Legion Ladies Auxiliary and although I have my birth fathers DD-214, they are requiring a copy of a birth certificate showing my last name the same as his.

Has anyone ever gotten a copy of their original birth certificate??? I'm so afraid that my birth mother only used her name when I was born, as she and my father were not married at the time. I have no idea what I can do to link my last name with my father's if that's the case. Any ideas???
