Problems with painted floor

The wood floor in my granddaughters' bedroom was in bad shape, so I decided to paint it. First I washed the floor with TSP, followed by a very light sanding. Then I primed it with a latex sealer/primer. I gave it two coats of Benjamin Moore Waterborne Satin Impervo in white, and then painted a decorative "rug" on it with acrylic craft paints. Lastly, I used a waterbased polyurethane. It's beautiful, but after about 10 months the areas beneath the cribs' wheels is peeling back to the dark wood of the floor. Also, the white background could use a fresh coat of paint.

Do I need to strip the whole floor to fix it? I sure hope not. Can I sand the areas that have peeled back to where the paint is tight to the floor, and then repeat the process? Should I use the same paint, or is there another type of paint that would be preferable? Did I do something wrong? Any advice will be greatly appreciated.
