Vintage recessed can light -- how to update?

I have a recessed light fixture in my living room that might be quite old. The housing is 6.5" in diameter and 7-8 inches deep. The light bulb sits way up high, far out of view. The trim is a thick ring that nearly covers the entire opening. The thing gives off almost no light.

How can I get more light? Should I replace the fixture, or could I add a baffle and different trim to the existing fixture? The housing is heavy and substantial -- appears well-made. But the unusual size has been wondering how I'd find a new baffle/trim. Or, if I did replace it, what I'd do about the large ceiling cut-out. What do you think?
