Please someone explain 'Black Tip'

OK, I've heard and read so many things about the causes of so-called 'Black Tip' (and inflo failure -- where it turns black and falls off). I've heard and read it's caused many things, including the following:

1) by a fungus,

2) by spider mites,

3) prolonged cold conditions,

4) prolonged wet conditions,

5) prolonged cold AND wet conditions,

6) improper conditions as plants come out of dormancy

7) An unnaturally long dormancy period

8) all or none of the above??

So which is it? Perhaps it's just a condition that appears as a result of 'stress' -- and stress can come from many different causes. Maybe any single cause or combinations of any of the above?? Are there any definitive studies on black tip? I'd be really curious if it's ever a problem in Hawaii or South America, or even way South Florida, -- areas where 'cold' and 'wet' are never a problem? I noticed in Jim Little's book 'Growing Plumeria in Hawaii' he doesn't even discuss it. So in Hawaii it may not be a problem. I would think any complete book on growing plumeria should address it thoroughly.
