why did all my pepper seedlings die?

I grew about 30 peppers from seed, starting in Feb. I didn't transplant from the 6 pack cells, until early May. I think that was my first mistake. They seemed so small, so I didn't think they were ready to transplant. When I began hardening them off, at the end of May, a lot of them died. But, some survived and I put them either in pots or in the ground. By early July, almost all of those died. The leaves would slowly fall off, until none were left, then they died. It was very sad. One revolution pepper survived and was doing very well in a 5 gallon pot. But, sadly, all of a sudden it drooped and died. All I can think of is that none of these plants ever formed strong enough roots. Can anyone offer any advice? Every one of the pepper plants I purchased, both hot and sweet, is thriving. Here is a picture of the revolution that just died.
