Best way to start a bed from scratch

I know there are several methods of making a new bed but I want to hear how everyone does it.

I have nice loamy clay (near a creek) near the center of OKC. Bermuda and Crabgrass are my biggest enemies. It seems like every year I start out with lots of mulch and good intentions but then the rain and the heat cause the grasses to take over the beds. With a full time job (and a husband) I don't have time to stay on top of them.

I want to completely remake my 12x14' vegetable bed this fall so that I have a chance against the grasses. Right now I have a border of treated 2x6 but it's just a visual boundary. The grass just doesn't care.

In addition, I need more room for vegetables. I've got squash and beans and stuff in the front flowerbeds because I'm out of room. So I want the new bed to start on the right foot.
