Winter-kill Limelight Tree Hydrangea

Last spring (2008) I ordered a Limelight Tree from that cute Philip Watson. They sell tons of Hydrangeas. While smaller than I expected, it leafed out and did very well all summer. I put some mulch around it come winter. This spring, the branches had green when scraped, but never budded out. Cottage Farms sent me a replacement with no questions asked, and the new one is a much better size. The old plant has a shoot coming out at the ground level. If I leave this to grow, will it make a decent plant? Will it make a tree form, or do I have to train it for that? Also, do I need to mulch more than I did? The winter was very normal except for warm weatehr early followed by more frost. I am disappointed as these do not bloom the first year after transplant, and now I have no blooms for this year!
