Trying to save this daylily plant

I've dug one of my DL's this morning because the leaves were going yellow.

I fear that I've waited too long to save the plant for its leaves were going soft at the base of the crown with the crown is suffering from rot also.

I removed the rot from the crown, which I know is very damaging to the plant.

Part of the crown and some of the roots are still firm with new grown from the roots.

I will plant into a pot in order to watch, but I fear it is a lost cause.

Any suggestions as to what to do next; or what I should not have done to the poor plant? Its the only plant in that area that is suffering from this. I suspect that too much dirt may have washed over the crown, for it was on a slope.
