A few questions on rose care

It's my second year growing roses and the four i planted last year came back and are doing quite well. I do have some questions however.

1. It appears that something is eating the leafs on the roses (multiple small holes in the leafs). Im currently using Bayer Advance Rose and Flower and it is supposed to protect from leaf cutter insects but my roses are still getting eaten. Any idea or suggestions?

2. When should i cease fertilizing to help them prep for winter (Zone 5a - Central Iowa)? I dont recall from last year.

3. For winter prep i corral the roses with chicken wire and stack leafs about 24" tall around the roses. It works well but i found the branches which made it through the winter only produce small leafs and dont flower much. The current year canes have large leafs and produce many flowers. Should i prune more in the spring to resolve this issue?

4. Any suggestions for powdery mildew control for roses grown in containers? Ive heard of a diluted milk solution; does it work?
