Nitrogen and various other things

First the soil analysis has a notation that is not clear about Nitrogen or rather nitrate - NO3-N #/acre. Then under the lime and fertilizer recommendations it gives the amounts to use.

Interesting that the amount of Nitrogen it double that for P and K. Well far less than that for lime.

This leads to a couple of other questions.(Oh by the way the analysis for both P & K were high) So why is more recommended? And finally what form of nitrogen is suggested? I read on the web that different ones react differently in the soil.
