Hi, daesa. I would cut off below the shriveled part, and keep the rest. Certainly, above your keikis (I'm assuming you want to keep them). About orchid media-- a lot of us don't live near orchid vendors, can't get good orchid media locally. We order on the internet. You have to pay for shipping, but it's that or nothing :( Carter and Holmes and Oak Hill both offer hobby size bags of different media and offer shipping at reasonable costs.

Your plant looks healthy enough. Nice growing. The old bloom spike at the tip of the cane indicates it is a intermediate type of den. It likes to be kept reasonably warm year round, like phals, and likes to be watered well and then allowed to almost dry. It likes as much sun as you can give it, more than phals but less than catts. Growth coming from the base is a good thing, will eventually be a new cane or pseudobulb. Growth coming on the canes is a sign of perhaps too much fertilizer at the wrong time, too much water at the wrong time, or too much nitrogen. Dens don't like a lot of nitrogen. Or it may be the type of den that keikis naturally. Your den threw out keikis in the fall because in the fall this plant needs to have fertilizer and water cut way back. I slow down fertilizer in August and water in September. Also, dens bloom better when the roots are crowded. So keep the plant in a small pot and let the pot fill with roots and then you may see some bloom.

Phal. Kuntrarti Rarashati is a primary hybrid of Phal. equestris and Phal. venosa. My blooms do have a light, pleasant fragrance.

If the plant isn't putting out leaves or roots at the moment it probably is a good time to re pot, I don't think the keiki would be affected. I'm not an expert either especially with phals but if the plant starts a growth / flower cycle it could be another year before you have a chance to re pot again.
