Containered Potatoes

First time growing potatoes was last year with Yellow Finns in 18x18x18 ish containers. I used good potting soil with a handful of 5-5-5 for about a foot of depth then used leaves with leftover peat (I won't be buying more) and compost. I didn't add any lime. I left 3 -5" of green while I built up using burlap bags as I had heard the potatoes grow up along the stems. I harvested soon after the flowers died. I only got about 8 or 9 potatoes for every two potatoes planted (two per container).

I am wondering what I did wrong. Did I need to add more soil? Should I have waited until all the above growth was brown and dried? Thanks.
