Best hazel cultivars for the home gardener?

I'm looking for input on which hazel varieties I should look into for home nut production as well as part of a diverse fruiting/nutbearing windbreak along our northern property line. American hazel grows wild here, and I do have a few I grew from seed already planted along the line, but I wanted to plant 5-10 hazels that put out significantly more nuts/plant than these would when mature. I'd also like varieties that grow 8-12' tall since that would be the perfect height for the windbreak/snowfence I'd like to establish. Right now I'm looking at plants from Badgersett Nursery in southern MN, St. Lawrence Nursery in New York, and Oikos Nursery in Michigan due to the fact that most European and hybrid hazel cultivars don't seem to be winter-hardy into zone 4 while theirs claim to be.

Can anyone recommend some other sources to try as well? Or, if there are some hardy, blight-resistant European varieties I could try? Thanks!
