To prune or not to prune a 2nd year GoldRush tree?

Last year I asked whether I ought to prune my GoldRush whip. Since I want the bottom tier to start high, and since, as Harvestman told me, GoldRush does not require much pruning, I did not prune.

Now I am wondering whether I need to this year or not. Bear in mind I am a novice, and, while I've read quite a bit about pruning there is so much contradictory advice it is rather daunting. So I thought I'd post pictures, share my fledgling thoughts, and let you all advise me.

The tree is about 6 or 7 feet tall. The lowest tier of three branches starts around 2 feet from the ground, about 3/4 way up the stake in the picture. The three branches are not evenly spaced around the trunk -- two are opposite one another and the third in between those two. Is it okay anyway for these to become the first scaffold?

About 18 inches above that are a whole bunch of branches. I was thinking one might eventually prune these down to four for the second scaffold? If this is correct, how does one go about choosing?

Then the top: the leader is crooked and there are another number of branches with lesser crotch angles all also reaching for the sky. What to do with those? And is it okay for the leader to be crooked?
