Strawberries in Maryland - Questions

I am considering growing strawberries for the first time in my raised vegetable garden beds. (soil pH 7.4, 10% OM, high nutrients)

Several questions -

1) bare roots vs plants?

2) the recommendations that I read are to plant in the spring at about 1 plant per square foot and then allow runners to populate the bed to about 5 plants per square foot by the end of the first year. They also suggest pinching off all flowers in the first year to allow bed/root growth. Could one simply plant 5 plants or roots per square foot and allow the plants to produce in the first year? I would then remove all runners that then rooted.

3)Best varieties for the mid-atlantic (also known this winter as the tundra) Maryland?

4) Best supplier?

5) Any other suggestions?
