Getting the most out of your bathroom vent fan

I've done a lot of searching the forums here and have come across a lot of good information concerning bathroom exhaust fans. One thing I haven't seen discussed here is airflow when using the bathroom fan. To get the most out of an exhaust fan should the bathroom door be closed with no gap under the door so it pulls only the air inside the room? Should there be a gap at the bottom of the door, allowing some fresh air in but mainly pulling air from the bathroom? Should the bathroom door be opened or cracked to create good airflow when the fan is on?
