Melissa, It appears to be a Graptopetalum paraguayense, or has some in it (there are lots of hybrids). There's a lot of information here already written on rooting cuttings (use the search box at the top of the main Cactus and Succulent Forum page), but basically, no water except for misting the cutting, the cut end slightly inserted in the soil, lots of light (eventually full sun, but not yet), warmth, air movement and it should be growing in a month or so.

That distance could be why...and also, it depends on lights you are using. I use lights for about 6mo/year. Plants are much closer, some as close as 2". I do not think any are further than 8-9". Others keep them further away. But I make sure plants are well acclimatized to good light. I use bulbs or tubes with high kelvins, most are 6500k, few bit lower. IMO, 4000k is quite low, I would look for min 5000k. And lights are directly above plants. They are all on timers, 12hrs on - 12hrs off.
