Painting woodwork

I've been mulling over painting my wood trim for some time. Right now, its a stain with polyurethane topcoat. I've held back starting because I know its a lot of work, but I really think painting it a nice antique white would brighten up the house a lot. I've painted cabinets, so I know the drill - clean, sand, prime, sand, paint, etc. How much sanding do I need to do, can I just scuff it up or does it need extensive sanding? I looked into just replacing all the trim, but it is costly, I have a lot of windows and doors.
