Septoria Leaf Spot three years in a row -- how to avoid this year?

I used to grow so many tomatoes -- tons to can and give away. The past three years I've ended up with only a few to freeze because the Septoria takes over. It hits the heirlooms and the hybrids both.

I've tried mulching and not mulching, cutting off the affected leaves and not cutting them, spraying with copper and with serenade and not spraying, spraying the cages with clorox solution, using new cages (though they were close to the old cages), planting in several parts of the garden so that not all the tomatoes were in the same row.

I think it may have come in on a store bought plant, so I've only grown my own seedlings the past few years. I almost never water, but if I do I only soak at the base. I plant 3 feet apart so there is plenty of air. I rotate solanaceae on 3 year rotation.

What could I be doing wrong?
