Insulating a Concrete Slab Properly

Hello! Any advice would be appreciated on this issue. I have a home built in the 50s with a sunroom that was converted to an indoor living space. We currently have carpet on the concrete slab, but we are remodeling the entire room and would like to add warmth by insulating under the carpet.

We have had a problem in the winter with condensation under the carpet on this north wall of sliders. We hope to help this by removing the bulk of sliders. We will be only leaving a slider on the far right, and putting large picture windows where the other three sliders are. Both side walls and wall under new windows will be sprayed with closed cell spray foam insulation.

We have heard various options for insulating the slab. Some have said rigid foam board directly on concrete with plywood on top. Others said sleepers with foam board, then plywood. We have also heard to put a vapor barrier before the sleepers.

Any advice would be greatly appreciated! Thank you!
