Difficulty helping a neighbor

I live in a elder and special needs Apt building. Most of us here, are both old and have special needs. We have a lady who lives alone and was in the Hosp. for a week because of a blood clot and she has nerve disease that i can't remember the name of. Divics or something like that, that affects her legs and walking. She always used a rolling walker. Friday she was discharged even after her telling them she is too weak to take care of herself. It took 4 of us to get her into her Apt. One lady who needs a walker herself to get around, Two fairly young guys, both with the use of one arm, and myself, and I can stand just a short time or walk a short distance. The lady has too much furniture in her Apt, and we had to move stuff, so hopefully she can get around with her walker, but it is still a very tight space for her to get around. We have been taking turns stopping in to see her and fix her meals, but she fell again last night and it took 3 of us to get her up. I wanted to call an ambulance but she didn't want to go back in the Hosp. I told the other lady neighbor who has been helping, that tomorrow we are going to start making phone calls until we find some city, county or state agency who can help this poor lady. She has one son in the area, but he is a over-the-road truck driver, and his job is in jeopardy and I think she has a daughter in some other state. All of us here want to help, but it is so difficult. We all have enough, taking care of ourselves and I have Hubs to take care too. I just don't know what else to do.
