Can Someone Explain This (Medical Question)

My friends DH is still in pretty bad condition, though there have been some improvements (more than they EVER expected). She usually sends me a text message when the get reports. Today I got one..and it really doesn't sound too good..but then what do I know. This is the message:

Well they said he hasn't been responding good to the stimulation..which is pain. And they turn his sudation? (her spelling not mine)for about 10mins. before they do it. the Dr. said he was "postering" when they do it and he said it was not a good sign, but he is going to give him a little longer.

My question is what do they mean he is "postering"?

Abnormal posturing is an involuntary flexion or extension of the arms and legs, indicating severe brain injury. It occurs when one set of muscles becomes incapacitated while the opposing set is not, and an external stimulus such as pain causes the working set of muscles to contract.[1] In competitive sport competition, the Fencing Response is forearm posturing that indicates moderate forces on the brainstem. The posturing may also occur without a stimulus.[2] Since posturing is an important indicator of the amount of damage that has occurred to the brain, it is used by medical professionals to measure the severity of a coma with the Glasgow Coma Scale (for adults) and the Pediatric Glasgow Coma Scale (for infants).

The presence of posturing indicates a severe medical emergency requiring immediate medical attention. Decerebrate and decorticate posturing are strongly associated with poor outcome in a variety of conditions. For example, near-drowning victims that display decerebrate or decorticate posturing have worse outcomes than those that do not.[3] Changes in the condition of the patient may cause him or her to alternate between different types of posturing.[4]

I worked as an Interior Designer in a past life - BC, 'before children' :)... So I know where you are coming from. In your initial post you posed your question in a very general and broad way which: 1. makes it difficult to answer and 2. indicated to some that you might be ill prepared to take on this challenge. Perhaps you were just trying to find out the different ways owner/builders were working on their projects? If you are seriously considering being your own GC then there are resources available to you on the internet and at the library. As dixiedoodle mentioned, The Owner Builder Book is one, it is great. There is a website as well. www.ownerbuilderbook.com I took a class at a community college and the website for the teacher is www.ownerbuilder.com. Tons of good information -that is how I found this site. :) We have decided to hire a builder but I think all of my research will still be an asset in our build. To answer your question about whether you can choose some of your own subs - it will depend on the Builder and your contract. If that is important to you then you should stipulate that and find someone willing to work with you on that basis. An option that might work for you is to find a GC/Builder that will get you through drywall and then you can GC the finish work yourself since you have contacts in the industry. Hope that helps.

This is for a skin cancer check. I would hint to them that since you have been a patient there before, that if they can't schedule an appointment without a referral, that you will go elsewhere...hoping that might be a possibility. Have you had skin cancer before or a history of cancer? BF had it several years ago, and then quit his yearly check-ups after his DR. died. He called the office to get an appointment, and they told him it would be a couple months before they could get him in. Once he let them know he had been a patient there some time ago, and was a cancer survivor, they found him an appointment time within a weeks time. If you call your DR's office, might they write you up a referral without the unnecessary Dr visit? Is that dermatologist a preferred provider or in your insurance network? If so, I'd sure be reporting the situation to my Ins so maybe they could get the problem taken care of, or drop them and find other providers. Sue

We bought a co-op there two years ago as an investment/pied a terre (all the regulars on this forum heard about it in probably too much detail). First, you have to pick yourself off the floor from sticker shock. :) Then, the process is much as Talley_Sue outlined. The big difference in NYC from other places is that you are required to use a lawyer at the closing. We also used an agent to find the apartment, so she took care of all the paperwork -- getting us preapproved, submitting offers, and so on. I believe she submitted offers in writing, but I'm not sure. There is no contract until the seller signs, so you can submit an offer, have it accepted, pay a lawyer for due diligence, and then if the seller doesn't send the contract back you still don't have the apartment (this happened to us on our first offer -- the seller was just testing the waters, I guess; it cost us quite a bit of money). With a co-op, after you have the contract signed by both parties, you have to apply to the co-op and have an interview. This process seems to take about a month but can take longer if the co-op board is not accommodating. Because we were using an experienced agent, all of our paperwork was done and in order, so there weren't any problems with that. In our case, the closing was then a month after the board interview. The board of our co-op (which is small, but the management is handled by a large established company) is very businesslike and there was none of the nonsense you sometimes hear about. Our first offer that fell through, btw, was for a condo. I thought it was interesting that they required an application and asked for all the same information that the co-op did, including letters of recommendation, although they didn't require an interview. I'm not sure on what basis, though, they could turn a person down, other than lack of financial qualifications. Hope that helps. There's lots of info on the Web about this.

Maybe she means they turn down the "sedation" (pain killers, like a morphine IV) to see how his response to pain is (like a pin prick to the bottom of his foot), and the response is the "posturing".

Today I got one..and it really doesn't sound too good.

I would conclude the same thing, and think that Sue's & others' interpretation is accurate. The wiki info seems applicable.

It is NOT a good sign.....usually seen in severe brain trauma.
At least that was what I learned when I was an ICU nurse long ago.
So sorry to hear about your friend's DH being so ill.

I don't know what your friends DH injury is from, but it is common in ICUs to keep people sedated, many times while they are intubated (on a ventilator) and put into a medically induced coma (sedation). The neuro team will assess a patient's responsive to painful stimuli after turning off the sedation. That stimuli is many times a sternal rub or skin twisting. It is a very bad prognostic sign to either not respond to painful stimuli, or to posture (turn the limbs out or in). Sorry to hear about your friend, but it doesn't sound good.

SleeperBlues..he was injured in a freak accident on Apr.28th. I posted asking for Prayers the day I found out about it.

I'm wondering if this is a 100%, beyond a shadow of doubt sign of a bad prognosis..or if it's still possible he could improve with minimal or no brain damage?

I don't want to be negative - but those are not positive test results and this is a really bad prognosis. "Minimal or no brain damage" is probably not possible.

Country Bumpkin thanks for the link-I haven't been here much lately and didn't know anything about the accident. Yes, it doesn't really sound good from what I have read and my experience working with trauma patients in the ICU. I hate to say this without sounding callous because Curt is not my family member, but sometimes death is better. It sounds to me that if he survives he will not be Curt as you know him, but rather in a vegetative state or something close to it. It is certainly a horrible thing to have happen to Curt and his family, and I pray for them all.
