Concrete OVER old pavers for patio?

Hubby and I have a 100-year old farmhouse that has an old patio out back (15'x16'); current patio surface is old rectangular pavers. We don't know exactly how long the patio has been there, but it's likely been there a LONG time-- 50 years or more?

The pavers are old and I'm sick of having to pull weeds from between them... I'd like to pave over the patio with concrete for a nice, smooth surface. I know we'll need to build a form and lay down wire mesh, but will we still need to lay down gravel over the pavers? (Or rip out the pavers and then lay gravel down?) Or can we just pour concrete right over the pavers?? (The patio area is about as "settled" as it's going to get.)
