Concrete patio replacement--remove old concrete or pave over?

We would like to replace our existing early 60's concrete aggregate patio in our backyard. It is accessible directly from the house from two sets of sliding glass doors. At it's highest grade the current patio starts about 1 inch below one of the sliding doors and slopes very slightly away from the house. We would like to replace the patio with either ungauged or gauged flagsone. Do we need to completely remove the old patio (appox 750 sq ft), lay in baserock, etc and pour a whole new concrete sub-slab? Or, to maintain the grade, just break out the concrete closest to the house, re-concrete just that portion (lowered) and then lay the new flagstone over the new and mostly old part of the concrete patio?

If we do have to completely remove the old patio, do we have to pour a new concrete sub-slab? Can the new flagstone be installed using baserock, sand, etc.?

I know this long--but this is the single most expensive portion of our landscaping estimate and have received varied opinions as to what to do. FYI, we live in oakland, california with moderate rain & the eaves extend from the house by about 3 feet. Thanks for any help!
