Water heater:Should thermostats be set same or different temps?

I just had to have my lower element replaced. Cost $75 for visit and $19 for element. When the element failed we noticed because the water temp was warm, not hot. I checked and discovered that the upper thermostat had been set well below the lower thermostat. While this allowed us to notice the failure, is this the recommended setting or should they be set to the same temperature?? If they are set the same how do you detect a single element/control failure? This is the first water heater that I have seen with seperate settings instead of one global setting.
