Gap between handle and plate in shower fixture

We are adding a bathroom and it's nearly done. This weekend I went to install the handle and trim for the shower fixture (Moen 82496) and ran into a minor but annoying problem. I've read and re-read the instructions and I *think* I've done everything right but there is a gap I can't get rid of between the handle and the trim piece that goes against the wall (see attached picture).

For those that do this all of the time:

1. Am I doing something simple wrong?

2. Did the licensed plumber (e.g. not me) who did the rough-in, position the valve incorrectly as related to the face of the finished wall?

3. Is there a piece missing or one that I need to purchase that would solve the issue? This looks like it could be right but I'm not sure (Moen 96945 Posi-Temp 1" Handle Extension Kit For 1 Handle Tub/shower).

The fixture works fine so I can live with this if I have to... but I'd rather not since it'll always bug me.
