Please help identify this bath faucet

I feel that my bathtub faucet is leaking within the wall. When I run my hand on the floor along the tub and get to the wall where the faucet is, the floor puffs up a little. When I have the faucet on, there is a slow but steady drip from the stem with the single-handle valve on it. The drip comes from the ridged joint where the stem meets the handle assembly. (You can see the ridged area in the picture).

The shower on the other side of the same wall rotted the wall with water saturation and hasn't been used since before we bought the house. (Unused since December '06.) Same faucet on that side.

How can I identify the model series of this faucet so that I can find a retrofit kit? The faucet is a 25 year old contractor-grade Delta. That's all I know. Can anyone help? Please?

Delta has made a model that looks like that for years. Inside you are going to find a ball with a stem, 2 rubber cup shaped seals, 2 springs behind the seals, a plastic cam and a rubber packing that goes under the platic cam. As far as I know there are only 2 versions of the springs and 2 versions of the seals. Most of the older balls are the same.
I would recommend calling plumbing supply houses and find one that carries Delta parts. Take you faucet apart and bring all the parts in a bag with you. If a supply house carries Delta parts, they will have what you need. A complet kit should be under $20 if it has a stainless ball and less if it has a plastic ball.
One good thing about Delta was that for many, many years they did not do much changing of the interal parts of their faucets. They still make one that looks exactly like the one in your picture.

Thanks for the info! When I take the faucet apart, I'll need to turn off the water main, I suppose. haha Will I need to reach down into the assembly to pull out each of these layers of parts, or will they all be fit together as a unit?

My condo was built in 1987 and I can't for life of me find replacement fixtures. My tub/shower has the following: 1 hot/1 cold pipe coming out. These are attached to the typical tub knobs but they are connected, there is a central shower pipe that goes back into the wall (and comes out on top shower head). This pipe is dead center in between the 2 knobs and its fixed. This is different because typically what I see is that the shower/tub switch is somewhere inside the wall but mine is external on the faucet and then it goes back into wall. Now typically these would have a free hose type shower where the water is diverted at the bath faucet but mine is hard wired to go straight back into the wall. Is there any hope in finding replacement hardware or do I need to re-plumb my bathroom? Any places online that might sell this hardware? What's it called? And I'm not interested in repairing seals, this hardware is plain ugly. Thank you for help

If you have to replace parts this is the step by step:
Turn off water. Open valve to drain out any water pressure in line. Put old rag or towel over drain to catch any small part you might drop. Remove cap on faucet with small screwdriver. Remove screw that you see. Remove handle...put straight off. Large chrome ring that you see will have serations around the edge. Remove ring by turning counter clock wise with large pliers trying not to marr it up too badly. With chrome ring off you will see either one or two pieces of white plastic. If one has littel gooves in the face of it, put a screw driver in a groove and tap gently in a counter-close-wise direction to remove. Then pry the white plastic piece (cam) out with small screwdriver. Remove rubber seal behind white plastic piece. Ball that has stem on it should pull right out. Look with a flashlight in the hole and you will see 2 rubber seals. Remove rubber seals with needlenose pliers or a small pick. Behind the 2 rubber seals are springs....remove those using same method. All parts are now out.
From the problem that you described I think you only need one rubber seal that's under the white plastic piece, but I would replace the 3 rubber parts and the springs.....about $5.00. The matching rubber seals and springs are normally referred to as "new style" and "old style." If you go to a place like HOme Depot, they will most probably only have one type. You are better off to go to a plumbing supply store that carries Delta parts and bring the parts with you along with your picture. If you plan to keep the faucet for many years, you may want to just replace everything as I described in my earlier post.
And to answer your question, you don't have to reach "down" into the assembly. Everything will be right there and you'll be able to see fine with a flashlight.
When reassembling just keep in mind that the white plastic cam has a little piece sticking out from the side. This little piece goes into a groove on the faucet. In other words it only goes in one way so don't force it.
