Toilet spec shorter than rough-in. Installation problem?

I want to buy and install a replacement toilet. I measured the rough-in, and I get 14 inches. The replacement has a 12-inch rough-in distance specified. My questions:

  1. The house was built in 1984. Why would they have intentionally used a 14" rough-in? It's got the original Mansfield toilet, a piece of junk IMHO, I think "builder's grade." The cover is about 3 1/4" away from the back wall, so it looks like it was intentional but wrong.
  2. Assuming I do indeed have a 14" rough-in,does it really matter having the new toilet 2" farther away from the wall than the manufacturer intends? I have no sense of bathroom aesthetics, but I'd hate for my better half to see the new installation and cringe.
  3. I've heard about something like an offset flange you can get to adjust for just that sort of problem. Is it sure to work, or does it depend on your toilet? I have seen something about a "skirted" toilet allowing more flexibility in installation than those that aren't.
  4. And what does it mean that a toilet is "skirted?"
