What the heck? White bean disaster

I make a white bean salad usually during the summer months. Today disaster struck again and I'm stumped (and ticked off).

I rinsed small dried white beans with cool water, covered with cold water to soak overnight. I did notice then that the beans looked more wrinkly and were losing some of their skins and wondered if they were gonna be THAT kind of bean....

Today I changed the water, set them to gently simmer and about 45 minutes later I ended up with a pan of bean skins, bean bits and a general mess. In fact, this is the worst I've seen, I had to dump them. I don't think there was a whole bean in the bunch.

I didn't salt the water. Nor did I let them boil, just a gentle simmer.

What the heck is going on when this happens? It has happened a time or two before, I just wonder why?
