Restaurant tip in cash - added to charge card bill

I finally took the time to compare saved restaurant tabs to actual charge card billed amount, and discovered that a nationwide chain restaurant had added $4.00 to charge amount, even tho I had left a substantial cash tip and marked a line through the tip part of the bill. I called the card co. and they credited my account for that amount, and advised me to write CASH in the tip part. After checking around, it seems that that chain is notorious for doing that. Also, it seems to be a practice at some establishments that if one charges a 15% tip, sometimes it is upped to 20%.

Have any of you had that happen?

We eat at that chain quite often and are going to confront the manager the next time we are there. The amount isn't that important, but theft sure is.
