finished kitchen people -- some advice please!

I posted a question about contract allowances and how they work with a more boring title so perhaps this will bring the people I need -- those who came to the end of their project!

Here's my question:

If the allowance is for something that is unknown at the time of the contract, because of the unknown nature of the extent of either labor and materials or both (say for patching tiles or flooring), how is that handled IF THE INCREASE IS NOT ATTRIBUTABLE TO A CHOICE MADE BY THE HOMEOWNER (e.g., more expensive tile)?

Is the homeowner entitled to any advance notice or request for approval (i.e., a change order) if the allowance is going to be exceeded?

Or, is the allowance just a rough guess and the number has no contractual relevance?
