Kitchen toe kick register

I am in the process of a kitchen remodel - same layout, new cabinets. There is a toe kick register under the sink. I took it off for the first time (after living in the house for 25 years) and realized that there is no duct from the floor to the register - the space under the cabinet was used as a plenum. I do not want to repeat that mistake with the new cabinets.

I went to the hardware store to find a transition piece (6" round to 2-1/4" x 12"). I found one, but if I attached it so the round collar was at floor level, the rest of the piece is 5" tall. A standard toe kick is 4".

What's the logical solution? Do I have my cabinet maker put a larger toe kick under the sink, to allow room? Or do I cut the floor to accommodate the transition, so the rectangular outlet is closer to the floor?
