Replacing basement slab -questions for contractor?

I want to replace the concrete floor in my garage/basement. It is 90 years old and very rough and cracked and stained. Can folks provide any information I should consider before talking to a contractor? For instance:

- how much time to allow a plumber to put in rough plumbing for a new floor drain (it's an easy connection to the main drain pipe)?

- should we expect a moisture barrier under the slab?

- would it be significantly more difficult to lower the floor 4-6 inches - would this require re-engineering the post footers and the foundation at the edges?
