This flashing must be wrong; please help, soon!

Our framer has never dealt with self-adhesive flashing before  oh, allready this sounds so not good! So, this is what we've mostly got:

As of now, all window flanges are on the outside of the flashing.

I've Photoshopped in what we THINK it should look like:

That's with all flanges  EXCEPT the bottom flange  inside the flashing. Is this close to right? If not, how so?

And, then, there's the round window problem:

with NO window flanges under the flashing. Should all the flange be under the flashing except the very bottom?

And, finally, flashing from the above deck:

Why oh why is the paper going behind the window? If this is right, it's the only place . . .

We have many many flashing techniques employed on this single house, but these are the most common. I'm requesting our supervisor to meet me on site tomorrow and could use some confirmation. Oh, this is so not fun.
