titmice babies: poor survival rate?

I had a titmouse lay 7 eggs, all 7 hatched. 2 chicks died early on (don't know why). The remaining 5 did well up until Tuesday when 1 of the chicks ended up on the bottom of the pile of chicks & I watched it die (this nestbox has a video camera inside of it). I think that one of its legs was stuck in the nesting material. The body is still in the nest. I hesitate to open the box to take it out until all the chicks are gone.

Yesterday 3 of the 4 chicks fledged, leaving one lone baby. It's calling for its parents but as of right now, they haven't come to feed it. It has been practicing flapping its wings but hasn't approached the hole.

Do titmice in general have a low survival rate? Would they purposefully starve a chick who isn't ready to fledge?
