Almost ready to transplant my tomatoes....but...

I'm not sure what I need. This is my first time growing tomatoes in the ground and growing them from seeds. Last summer I grew one plant in a container and it didn't do so hot.

So, my question is what do I need to grow my tomatoes in the ground? The soil in my yard doesn't look that great to me, so I'm thinking about putting down top-soil or compost before I plant the tomatoes. Which would be better choice? Also, I've read that tomatoes like warm soil and it is a good idea to put down black plastic mulch. Is this true? Right now I have starts growing in the little trays. All of the starts have small true leaves, should I transfer them to bigger individual containers or plant them directly in the ground? I live in Indiana if that helps.

Sorry for the barrage of quesitons, but I want to do this right and I want lots of tomatoes! thanks in advance for your help!
