PeeGee Tree

Hi,

I have a very old tree-form hydrangea growing in my backyard. It has three trunks each about 2 and a half inches thick. The plant/tree has grown to be about 15 feet wide by 13 feet tall, and its "umbrella" is about 5 feet off the ground. This height has caused its lower branches to die off and produce drastically smaller blooms. The upper branches (as of last summer) continue to have the large blooms though.

How should I prune it so then the lower branches become more prolific, and how could I get to the top?
