Whats wrong with my lemon tree?

I'm a newbie gardener, This is my first post in this forum.

I bought a Meyer lemon tree from the nursery 4 years back. I planted it next to my lawn and it appeared to grow well for a year. After that it started loosing leaves, flowers occasionally. And in a year I get may be one or two fruits that are very hard.

There is only one branch with leaves, I still see few leaves in the branch that has leaves. Rest of the branches are bare. The leaves seem to be deformed. Two weeks back, I noticed something black on the underside of the leaves, suspecting spider mites, I applied Safer brand Insect killing soap to the leaves. I fertilized the plant two weeks back with Jobes Organic citrus fertilizer. And plan to fertilize once a month (is that too frequent?). I also added Dr.Iron iron supplement when I fertilized two weeks back.

What am I doing wrong? Is there anything that I can do to revive this tree?
