Jade branches mushy and falling off

I have a jade plant that has been on the decline for quite a bit of time now that I am trying to revive.

I have read on this forum that most of the problems come from overwatering. I have attempted to water him very sparingly recently (once a month), but the thick branches are mushy to the touch. Others fall off although the leaves seem healthy and the branches are much more solid than the mushy ones still stuck to the plant.

I am also afraid of the plant possibly not getting enough light. I have a west facing window (only option in my apartment), and in the warm months it is blocked by another tree. Should I try artificial light?

Am I watering incorrectly? I have taken the plant out of its pot and cleaned the pot with warm water and mild soap. I have read to make sure you get specific soil that drains properly. Is my pot set up incorrect and not letting it drain thoroughly? I'm not sure if it is root rot so I can try clipping some of the roots. Should I get a completely new pot as well?
