Removing sucker below the soil line? Crown or sucker?

Hi! Thanks for reading my question and taking the time to look at pictures. I know that self-waterers are not good and so am going to take it out when I do whatever is suggested for the following questions...

I am wondering if my AV has two suckers or just one. It seems to have a sucker that started below the soil line. I thought it had a baby, lol, until I got to reading on here that that can happen. But so far I have not run across how to take a sucker off that has started below the soil line. Any suggestions? Here's a pic of it.

Secondly, I'm not sure how to tell if the small leaves in the center are just the crown or another sucker. The leaves all seem to be in line with the crown...so maybe not. But since I'm gonna be doing something to my AV I might as well do all at once. A side view and top view....

Oh yeah...the AV as a whole is very unstable. It practically died a few years ago and basically came out of the soil without roots. I put it back in the soil hoping it would take and it did.....but it can be wobbly. What should I do about this?

And just a quirky side note. I am fairly active on Flickr (photo web site) There must be a lot of AV lovers on there. I posted pics earlier for another question and wasn't expecting any views.....but got over 40 in two to three hours!
