Moss on bed, and poor growth

Happy new year!

Any advice gratefully received from the forum mind.

I constructed a raised bed 2 years ago. Original soil was heavy clay but i have since added compost, manure, shredded loam, etc, and picked a lot of stones. Year 1 I grew mainly leafy vegetables, year 2 potatoes. The yields seemed reasonable.

After I pulled the potatoes in the summer I sowed more leafy veg, but had poor germination rates. I hoed what was left in and sowed again, this time (because of the season) erecting a rudimentary polytunnel over the bed. Again, very little has come through, and two months on I've noticed quite a lot of moss growing on the surface of the bed.

Any ideas about what I am doing wrong? I suspect the soil may be sour but I don't know how to check this out or how to put it right.
