It is a penny round, sourced from Ann Sacks in their Savoy collection, found here: https://www.annsacks.com/products/tile-stone-mosaic/savoy-mosaics#/savoy-penny The Lantern color is the main penny round color seen, and the Lotus color was used for inserts (matching the bottom section 4x8 tiles) - they are both a blue-ish grey tone, with the Lantern being the deeper color. And you are correct that this was done by hand!

No. Tile gone wrong.
The floor is the unglazed ceramic mosaic line by Daltile. These are the 1" hexagons in white, which come mounted on mesh. Have the tile installer pop one out and replace with black to create the pattern. We used a medium gray grout color. Hope that helps!
